What a Chronic Disease Management Plan Covers
A chronic disease management plan is prepared with your GP when you have a chronic or complex condition that is expected to last six months or longer. This can include conditions such as asthma, coronary heart disease, chronic obstructive pulmonary disease (COPD) and arthritis.
Your chronic disease management plan sets out how your condition will be monitored and managed over time. Where appropriate, it may also provide access to Medicare-subsidised visits with allied health providers as part of your ongoing care.

To find out whether a chronic disease management plan is appropriate for you, book a longer appointment with your GP to discuss your condition, medical history and current care.
Your GP will assess your eligibility and, where appropriate, prepare a chronic disease management plan based on your ongoing health needs.

Chronic disease management consultations are bulk billed for eligible Medicare cardholders, including Pension Card holders, Health Care Card holders, DVA Card holders, children under 16 and patients undertaking a Mental Health Care plan.
