Type 2 diabetes treatment is individual to each patient and may include lifestyle changes, regular monitoring and medicines where clinically appropriate.
Diagnosis and Testing
Type 2 diabetes can be diagnosed using blood tests, with your GP advising which tests are appropriate for you.
Kilcoy Medical Centre has 4Cyte Pathology on site, allowing pathology ordered by your GP to be collected at the medical centre during the pathology collection centre’s operating hours.
Blood tests used in diabetes management can include blood glucose and HbA1c testing.
Ongoing Monitoring and HbA1c Checks
Ongoing diabetes management includes monitoring how your blood glucose is tracking and reviewing your treatment over time.
HbA1c is a blood test that reflects your average blood glucose over approximately the previous 8 to 12 weeks. Your GP will advise how often it should be checked based on your results, treatment and individual circumstances.
Regular reviews also give you an opportunity to discuss any changes in your health, treatment or diabetes management with your GP.
Type 2 Diabetes Treatment and Chronic Condition Management Plans
Type 2 diabetes treatment can include changes to diet, physical activity and other lifestyle factors, together with medicines where clinically appropriate. Your GP will recommend treatment based on your health history, test results and individual needs.
Patients with diabetes may also be eligible for a GP Chronic Condition Management Plan if their GP considers that a structured plan would help manage their condition.
GP Chronic Condition Management Plans replaced the former GP Management Plan and Team Care Arrangement system from 1 July 2025. They are available for eligible patients with at least one medical condition that has been, or is likely to be, present for six months or longer, or is terminal.
Where multidisciplinary care is appropriate, a plan may also provide access to Medicare-supported allied health services.
Your diabetes management may also include regular health checks commonly referred to as the annual diabetes cycle of care. Your GP can advise which checks you need and how often they should be completed.
Lifestyle Support and Referrals
Diet and physical activity are important parts of type 2 diabetes treatment alongside medical care.
Where appropriate, your GP can refer you for additional support. Kilcoy Medical Centre’s Allied Health services include dietetics provided by Healthy Lifestyles Australia.
Costs
Kilcoy Medical Centre is a fully bulk-billing practice for eligible Medicare cardholders.
Some services are not covered under the Medicare Bulk Billing Incentive Program and may attract a private fee. See the fees and billing page for current information or check with reception before your appointment.

Collection is available during the pathology service’s operating hours, so check current collection times if you intend to have your blood test completed on the same day as your GP appointment.
Diabetes management GP consultations are bulk billed for eligible Medicare cardholders at Kilcoy Medical Centre. Some services are not covered under the Medicare Bulk Billing Incentive Program and may attract a private fee, so check the clinic’s current fees information or ask reception if you are unsure what applies.
You do not need a referral to book a GP appointment at Kilcoy Medical Centre to discuss diabetes. You can make an appointment directly with one of the clinic’s GPs for assessment, testing, monitoring or ongoing diabetes management.
